In thirty years of buying selling, collecting etc I have never come across or know anyone else who has seen an issue 2 board.
@Andy You seem to have omitted either a link or picture of board You mentioned.

How do we know there was an Issue 2? Or maybe Issue 3 is really Issue 2?

The name of the version is printed on the board.
My Issue One is written out in characters and very lightly to see (see example) and has the hand routed board.
My Issue Three board is printed with ISS 3 and has same color (green).
Printing name on the board is always on the solder side, near the RF modulator box (left of it), there is much unused space.

As Pokemon rightly says, on bottom cover removal the issue and date can be found as described it should be noted , however , that most issue threes also have the date and issue number on the component side near the regulator heat sink and many issue ones have the issue number printed just above the CPU.
